The information we are requesting on this form will be used to create a master list of Performing Ensembles, which will be shared on the website. This resource list will help the public to find ensembles in their regional areas and foster ensemble collaboration.

The ensemble director or primary ensemble contact should answer the survey (not ensemble members). Please submit only one form per ensemble.

Please note that once this form is submitted it must be approved by an HMA team member. Your submission will not be viewable immediately on the website. Do not submit a form more than once.
